The borderline patients have splitted self and object representations. They also do not have independent identity since they could not achieve separation-individuation process. So they are very impulsive and can not have persistant object relationship. The therapeutic goal of borderline patients is gaining dynamic insight and object constancy. However, therapists inevitably meet the various difficulties during the dynamic psychotherapy. They are the issues of mentalization, regression due to ego weakness, violation of boundary and misunderstanding of interpretation as a critique. The charateristics of transference is usually psychotic and hardly resolve. Borderline patients frequently extrnalize their internal object relations in the consulting room. The countertransference is the most serious difficulty of them. The defense mechanism of projective identification play a major role in developing countertransference. The author reviewed those difficulties and suggest the way out of them.
